I suspect HorribleSubs, a popular encoding group, would take an MP4 file from Crunchyroll and use MKVtoolnix, a program for making MKV files, to add English subtitles.

However, modern anime on western torrent sites are usually English translated, meaning they are bundled with multiple audio tracks for dubbing or soft subtitles for at least 1 language. That's why MKV files always have soft subs or multiple channels for audio (multiple video channels is possible but no one does this that I'm aware of). MKV is short for "Matroska" which is a multimedia container, meaning a single file can hold an infinite number of certain file types: video, audio, and subtitle. None of these comments explain what it is comprehensively.
